Please read before completing your purchase of the WMHMDA Compliance Pack

These terms explain exactly what you're buying, how it was generated, and the limits of what a self-serve document generator can do for you. By checking the box in the checkout modal, you confirm you've read and understood this page.

1. What you are buying

The WMHMDA Compliance Pack is a set of five (5) draft documents (Consumer Health Data Privacy Policy, Opt-in Consent Form, Valid Authorization to Sell or "not applicable" record, Data Retention & Disposal Schedule, and Consumer Rights Request Form), generated in your browser from the answers you provided, formatted to reflect the requirements of Washington's My Health My Data Act (RCW 19.373). You already previewed the full text of every document, free, before paying — payment unlocks a clean, watermark-free PDF and an editable Word file of each.

How your files are delivered. Your documents are generated and exported entirely in your own browser. No file is emailed to you automatically, and no copy of your answers or documents is stored on our servers. After checkout, Gumroad emails you a receipt containing a licence key. Entering that key on this site unlocks your PDF and Word downloads. Please keep the key — you can use it to unlock your files on any device, at any time, and we recommend saving it with your compliance records. If you can't find the email, you can retrieve your purchase and key from your Gumroad library.

2. This is a template generator, not a law firm

WMHMDA.com is a self-serve document-assembly tool. It is not a law firm, and using it does not create an attorney-client relationship. The documents are drafts built from the categories, sources, purposes, and answers you provided — they are a starting point, not a substitute for review by a licensed attorney familiar with your specific business and jurisdiction. Laws change; you are responsible for keeping your documents current and for verifying they match your actual data practices before you rely on them.

3. All sales are final

Because this is an instant digital download and you are able to read the complete text of all 5 documents before paying, all sales are final once checkout is completed. We do not offer refunds for buyer's remorse, for a documented business change after purchase, or because a document needs further customization — you can regenerate your documents at any time using the same tool if your answers change. If a technical error prevents you from receiving your files (e.g., a failed download, or a licence key that won't validate), contact us and we'll make it right — please get in touch before disputing the charge, as we can resolve delivery problems directly.

4. Accuracy of your answers

Your documents are only as accurate as the answers you provide in the 7-step questionnaire. You are responsible for reviewing every generated document for accuracy against your actual data collection, sharing, and sale practices before posting, distributing, or relying on it.

5. No warranty of legal compliance

We built these templates from the text of RCW 19.373 in good faith, and we keep them updated as our understanding of the statute evolves. We do not warrant or guarantee that use of these documents will result in full legal compliance with WMHMDA or any other law, or that it will prevent an enforcement action, demand letter, or lawsuit. Compliance depends on your actual business practices, not just the documents you hold.

6. Your license to use the documents

Once purchased, you may use, edit, post, and distribute the documents for your own business's compliance purposes without attribution. You may not resell the templates themselves as a product or service to third parties.

7. Limitation of liability

To the maximum extent permitted by law, WMHMDA.com and its operators are not liable for indirect, incidental, or consequential damages arising from your use of, or reliance on, the generated documents, including but not limited to regulatory penalties, private lawsuits, or business losses. Our total liability in any case is limited to the amount you paid for the Compliance Pack.

8. Your data stays on your device

The questionnaire runs entirely in your browser. Your answers are stored in your browser's local storage and are never uploaded to our servers. We do not see, collect, or store your business details, and there is no account, login, or database on our end.

9. Educational content on this site

The guides, comparisons, FAQs and state-by-state summaries published on this site are general information for business owners, written in plain English rather than statutory language. They necessarily simplify provisions that contain exceptions and definitional detail, they may not reflect amendments or court decisions made after a page's last update, and they are not legal advice — reading them does not create an attorney-client relationship. Where we describe another state's law, or compare approaches such as hiring an attorney versus using a generator, we are offering general orientation rather than a recommendation about any specific provider, and any cost or timing figures are indicative ranges rather than quotes. The generator produces Washington (RCW 19.373) documents only; it does not produce documents for any other state or framework.
